Heart Failure Clinic

The Heart Failure Clinic is an outpatient service that is meant to help heart failure patients understand and manage their condition. This leads to decreased hospital admissions and emergency department visits for heart failure patients.

Each patient visit consists of a full body assessment, weight management, and education. Depending on the patient’s individual circumstance, the visit may also include medication titration, lab work, and administration of medication as needed. Patient care is provided by a group of specialists that may include nurses, social workers, dietitians and/or pharmacists.
